Donor Sperm

Before you place an order:

1. Your physician MUST be registered with Cryogenic Laboratories, Inc. before you may place an order for donor semen. Client Services is able to verify that your physician already has an established account. Client Services is available  Monday- Friday from 7:00AM EST - 6:00 PM EST (1-800-466-2796)
2. Please be sure to read and understand our Terms of Use which explains what to expect from our donor program and what we expect from you when purchasing and utilizing our services. The Terms of Use Letter must be signed and returned to us prior to shipment of any donor specimens. The form may be returned by fax, mail or scanned and emailed to info@cryolab.com. Contact Client Services for assistance if needed

• ID Option Patient Agreement (consent is at the bottom of the page linked here)
• Identity (ID) Option Birth Registration Form - must be completed after the birth of a child from an ID Option donor in order for your child to have the option to obtain donor information on or after their 18th birthday. For pregnancy by an ID Option donor, we encourage you to fill out the Pregnancy Reporting Online Form but it is not a substitute for completing this registration paperwork after birth. You can call client services (800-466-2796) to assist you.
